AGREEMENT, WAIVER & RELEASE

CONSENT OF PARENT/GUARDIAN (IF, PARTICIPANT IS A MINOR)

As the parent or legal guardian of the participant(s) listed below. I hereby consent that the participant may participate in intuitive piano and art activities with Cindy Blaser. I hereby execute the Agreement, Waiver, and Release on his/her behalf. I hereby state that the participant is physically able to participate in intuitive piano and artistic activities. I hereby agree to indemnify and hold the person and entities mentioned above free and harmless from any loss, liability, damage, cost, or expense that they may incur, as a result of any injury, or property damage that said participant may sustain while participating in activities with Cindy Blaser.
